To register your family with Fair PharmaCare you need:
for all family members:
- Enroll in the Medical Services Plan of B.C.
- Personal health number – found on your B.C. care card
- Birth dates
for you and your spouse:
- Social Insurance numbers
- Net income from Line 236 of your income tax return from 2 years ago
- Deduct the amount of Universal Child Care Benefit payments you received
